What do you need to bring on your first visit?

Please bring any removable appliances such as partials, dentures, retainers and night-guards you use. We’ll also need:

• Insurance cards and a photo ID

• Complete information about your medical and dental health history

• A list of your current medications

• ​Please have your old x-rays (taken within 24 months) emailed to our office from your previous dentist. Our email ID is info@plumdentalcare.com

• A way to pay for co-pays and out of pocket costs
